A Florida NEMT startup can mean an ambulatory sedan, a wheelchair-accessible van, a stretcher-capable service, a Medicaid-network provider, a private-pay operator or a company built around facility contracts. Those are not the same cost structure.

No universal startup total is published here.

Any unquoted vehicle, insurance, local fee, credentialing expense or operating assumption remains a placeholder. Verify the current fee directly with the responsible authority or provider before committing capital.

Why county choice changes the budget

Florida Statute 316.87 expressly refers to providers licensed by a county or operating under a county permit. It does not establish one statewide NEMT startup fee.

Two current official packets show why local costs cannot be generalized:

  • Broward County: its NEMTS license packet revised July 28, 2026 lists a $741 license application fee, a $369 license fee and a $75 vehicle permit fee. The separate 2026 vehicle-permit form also states $75 per vehicle. These are Broward figures, not Florida-wide figures.
  • Orange County: its official paratransit instructions list a $200 application fee including application processing and up to three vehicle inspections, plus $30 for each additional vehicle over three. These are Orange County instructions, not a statewide schedule.

Even when an official packet is current, confirm the fee and applicable service category immediately before filing. See the Florida NEMT requirements by county page for the classification and authority questions that come before payment.

The cost gates to clear before buying a vehicle

  1. Define the service

    Document ambulatory, wheelchair, stretcher or other planned transportation and whether any medical care could move the service into a different regulatory category.

  2. Confirm the operating authority

    Get the county or city to identify the current license, certificate, permit and inspection path in writing. A business entity filing does not answer this question.

  3. Obtain an insurance quote

    Quote the actual vehicle type, drivers, territory, business use and certificate requirements. A personal-auto estimate is not a commercial NEMT budget.

  4. Verify the payer pathway

    For Medicaid-related work, identify the member’s plan or applicable AHCA pathway and the current vendor/provider process. For facility or private-pay work, obtain actual contract and payment terms.

  5. Size working capital

    Model monthly cash outflow before relying on trip volume or payment timing. Neither is guaranteed by a license, vendor application or lead source. Use the free Florida NEMT Unit Economics Calculator to stress-test your own trip, deadhead, fixed-cost and payment-lag assumptions.

A quote-backed working-capital method

Build a monthly cash-flow schedule rather than multiplying an internet “average trip rate” by an assumed trip count. Separate fixed costs from per-mile and per-trip costs, then stress-test:

  • fewer trips than planned;
  • deadhead and return miles;
  • wait time, cancellations and no-shows;
  • vehicle downtime or an insurance deductible;
  • credentialing that takes longer than expected;
  • payment terms that differ by contract.

The result is your planning range, not a guaranteed startup cost. Pair it with the 10 checks before buying an NEMT van and the free 15-point pre-launch checklist.
